HWC Engineering offers a comprehensive medical and prescription drug plan that is available to all Full-Time employees and your dependents up to age 26. The cost of the medical plan effective January 1, 2024 on a “per pay” basis is as follows:

Your 2024 Benefits:
Our medical plan is a Consumer Driven Health Plan. A Consumer Driven Health Plan allows you to open a Health Savings Account (HSA) in which you can contribute pre-tax dollars. Similar to a Flexible Spending Account, the funds that you contribute to an HSA can be used for out of pocket medical expenses. However, unlike a Flexible Spending Account, you do not have to spend the funds by the end of the year……..if you don’t use the money, you can roll it over for use in future years.
HWC utilizes BMO Harris Bank as the bank for our Health Savings Accounts. Employees may direct monies from payroll directly into their HSA account on a pre-tax basis.Employees are responsible for determining if they are eligible for an HSA. Be sure to read the eligibility rules below for assistance on determining your eligibility.
In order to apply for a Health Savings Account with BMO Harris Bank, complete the application that is included in your new hire packet and either turn it in to Human Resources or turn it in to a teller located at the BMO Harris Bank branch that is located in our downtown Indianapolis building.

Pharmacy thru Epiphany RX
Epiphany RX handles our prescription drugs. Epiphany has a very comprehensive website that you can access that gives you a wealth of information as well as forms that you might need. Go to www.epiphanyrx.com and click on the RESOURCES tab for any of the following documents:
- Mail Order Prescription Form – use this form if you want to set up your prescriptions to be mailed to your house. The mail order pharmacy is Costco.
- Formulary List – this is a list of covered medications under your plan
- Prescription Reimbursement Form (if you don’t use your ID card at the time of the fill and need to file a claim.
- Preventive Care Prescription Drug List – medications on this list are covered at 100% and are not subject to the deductible
- ACA Mandated Prescription Drug List – these medications are mandated by the ACA law and must be covered at 100% with no cost share
- COVID Home Test Reimbursement Form – if you have purchased a COVID home test kit and need to get reimbursed for it, use this form

UMR gives you 24/7 access to your benefits and claims. To access claims processed, you must register and log on to the following website: To find a participating UMR provider, click on the UMR website link above and click on FIND A DOCTOR. Enter the requested information (type of doctor, zip code, etc.) and then choose the United Healthcare Choice Plus network when prompted. Once you have your UMR ID card, you will be able to register on the UMR website and have access to claims, prescriptions, treatment costs and other helpful information. |
My Benefits
Your UMR plan allows the opportunity for Teladoc provides you the opportunity to have a video visit with a board-certified doctor or licensed therapist on your smartphone, tablet or computer. Device must have a web cam.
Teladoc is a Choice Plus network provider and will file a claim to your UMR plan — just as if you visited the doctor at his/her office. Your cost at the time of the visit will be around $50……….and this $50 will go toward your deductible, if applicable.
If your symptoms do require a prescription, doctors at Teladoc can write a prescription send it to your pharmacy of choice for pick-up.

My Disability Benefits
HWC Engineering offers employees the opportunity to elect short term disability coverage through One America and have the cost payroll deducted. If you are not able to work due to a short term accident or illness, your disability benefits pay you a percentage of your income to cover your personal expenses.
HWC Engineering offers long term disability coverage through One America at no cost to you. Long Term Disability begins after your short term benefits are exhausted.
My Life Insurance Benefits
HWC Engineering offers employee basic life coverage through One America at no cost to you! One America offers a benefit equal to 1 times your annual salary. The minimum level of coverage is $150,000; the maximum level of coverage is $250,000 . The plan includes an Accidental Death and Dismemberment policy with the same coverage amount as the basic life plan.
My Voluntary Life Insurance Benefits
HWC Engineering offers employees Voluntary Life coverage through One America in addition to the employer paid Basic Life plan. This enables you to purchase additional coverage for yourself. If you elect to participate, you can also purchase coverage for any eligible dependents.
You can elect coverage up to 5 times your annual salary or $500,000 (whichever is less) for yourself. Coverage is available in $10,000 increments. During your initial enrollment, you may elect up to $150,000 without having to answer any medical questions. If you need to elect more than this amount, you just need to submit an Evidence of Insurability Form for underwriting to approve the amount over $150,000.
You can also insure your spouse for up to $250,000, but not to exceed 100% of what the employee elects. Coverage is available in $5,000 increments. Spouses who elect up to $30,000 can do so with no medical questions; however, amounts over $30,000 must be approved after review of a completed Evidence of Insurability Form.
Your child(ren) are also eligible for an amount up to $10,000.
My Dependent Care Benefits
Employees have access to a comprehensive cafeteria plan, which includes the following pre-tax benefits:
- Premium Only Plan – employees can elect to have their payroll deductions pre-taxed.
- Dependent Care Spending Account – employee can set aside up to $5,000 per year for expenses related to day care for eligible dependents. Eligible dependents are listed on page 19 of the Summary Plan Document (see below).
